The mission of the Baltimore’s City Health Department’s Field Health Services (FHS) program is to provide Non-Emergency Medical Transportation (NEMT) to and from medically necessary medical appointments for recipients of Medical Assistance in the City of Baltimore.
The FHS/NEMT program bridges the gap for Medical Assistance recipients by providing transportation to and from medically necessary appointments for Medical Assistant recipients as a last resort resource.
Eligibility:
In order to receive transport you must have:
- A medically necessary medical appointment
- Full active Medical Assistance Coverage
- Live in Baltimore City
- Have no other means of transportation available (Medical Assistance Transportation is the last resort)—
- no car or unable to drive,
- no family/friends who can drive you,
- do not live on a bus route or Mobility Transport route, and
- not eligible to ride with Mobility Transportation Authority (MTA)
The Maryland State Department of Health requires medical certification for all recipients. The Provider Certification Form (PCF) (attachment below), must be thoroughly completed by a designated medical provider. The original PCF must be signed by the patient’s designated medical provider and mailed to the Baltimore City Department of Health

Hours of Operations: Monday through Friday from 7:30 am to 5:00 pm.
How to schedule transport?
- Call 410-396-7633 Monday through Friday from 7:30 a.m. to 5:00 p.m. and follow the prompts.
- Call at least 48 hours two business days) before the appointment time. Transport reservations will be accepted until 3:00 p.m. Example: If you have a Monday appointment schedule your transport on Thursday before 3:00 p.m. If you have an appointment the day after a holiday, schedule your transport two business days before the holiday.

Please have the following information available when scheduling a transport
- Medical Assistance card number
- Name and address of the medical provider
- Appointment day and time
Program Rules:
- Escorts: An escort is allowed, if medically necessary, as space is available. The escort must be 18 years old or older.
- Trips: A trip is considered one way. For example, a round trip would be considered two trips.
- Curb to Curb Service: Drivers are not allowed to enter the client’s home or medical provider facility
- Wait Time: Drivers are only required to wait 5 minutes beyond the scheduled pick-up time
What if I need to cancel the transport?
- Please call 410-396-7633 by 3:00 p.m. the day before your scheduled appointment.
- It is important to cancel your scheduled transport as soon as you know that you cannot go to your appointment. When you cancel early, someone else in need may be able to be transported.
